Here is the latest RCMMORF (Red Card/Matchday Misconduct Offence Form) that you will need to complete asap after the game - click here. By the way, you should speak to Roy Gittens (023 80 255 892), as the Trojans Hockey Club Disciplinary Officer, as soon as possible after the game, probably after completing the form and before emailing it. After completion, the form must be emailed within 24 hours of the incident to both:
